ADULT FOSTER CARE IN WORCESTER, MA

More support. Still your life.

Daily care in a familiar home, with guidance for the people who make it possible.

Home is where life happens.

The morning routine. A favorite meal. The people who know you best. Adult Foster Care helps make room for support within everyday life.

In the Massachusetts Adult Foster Care (AFC) program, an eligible person lives with a qualified caregiver in a private home. The caregiver helps with daily activities, while nursing oversight and care management support the arrangement.

AFC is designed around a person and a shared home. It is different from a nursing facility, an adult day program, or occasional home-care visits.

COULD AFC BE A FIT?

Start with your situation.

MassHealth describes AFC for eligible members age 16 or older who need daily assistance with personal care and meet the program’s clinical and other requirements. The individual and caregiver must live together. A spouse or legal guardian cannot serve as the paid AFC caregiver.

Eligibility, caregiver suitability, coverage and enrollment require review. A website cannot determine approval or promise payment. Our team can discuss your circumstances and explain the next steps.

CARE IN A SHARED HOME

A familiar setting.
An individual approach.

The member and a qualified caregiver live together in the member’s home or the caregiver’s home. The caregiver may be an eligible relative or a person outside the family.

The arrangement is more than an address. An assessment considers the person’s needs, the caregiver’s role, and whether the home can support the care plan.

Participation depends on coverage, clinical eligibility and program approval. A first conversation is a way to learn about the process.
