Career applications
If you submit the career form, it collects your name and contact details, address, selected opportunity, work eligibility answers, employment and education history, professional credentials, reference contacts, availability, referral source and optional notes. It also records your acknowledgments, typed signature, submission time and the applicant statement you accepted. Please obtain permission before providing a reference’s contact information.
Your submission is stored in a private website recruitment inbox, accessible to authorized Worcester AFC staff after sign-in. It is used to review your interest and communicate with you about recruitment. The form does not make automated hiring decisions.
Notifications to the team
Worcester AFC may send your application details and a PDF copy by email to its authorized HR and leadership team for recruitment review. A private copy is also retained on the website. Access to recruitment records is limited to authorized staff. Email delivery and hiring follow-up are separate from the on-screen confirmation that an application has been saved.
What not to submit
Please do not include Social Security numbers, dates of birth, driver’s license or other government ID numbers, identity documents, criminal-history records, medical information or information identifying a person you have cared for. Professional license and certification details may be entered in the designated qualifications section. Résumé and document uploads are not requested by this form. Background screening, identity verification, clinical forms and other onboarding paperwork are handled separately by the team when appropriate.
Cookies and technical records
The application uses a temporary session cookie to protect submissions. Your answers are not intentionally saved in browser storage between visits. Hosting infrastructure may record technical request information for security and operation. The application’s rate limiting uses a hashed network address rather than storing the raw address with your submission.
No advertising trackers or third-party analytics have been added as part of this website design. External links, such as directions, lead to services with their own privacy practices.
Access and retention
Application information is retained for recruitment administration and related recordkeeping. Contact the team if you want to correct information or ask about access, retention or deletion; requests may be subject to applicable recordkeeping obligations.
General contact and referrals
Ordinary email is intended for general questions. Please call for instructions before sharing health records, member identifiers or other sensitive documents. The website does not collect clinical referral files.
